Notes about the Accident Form: Team Managers, keep a number of blank forms with you at all games. Forms should be kept on record with the Team Manager and shredded after last game of the season. The League Safety Officer keeps the original copy on record per Safety Director League Operations Manual.

Notes about the Coach Appraisal form:
All forms are kept confidential, shredded end of each year elections in September. All feedback is constructive; SRNLL is primarily an Instructional League. The Coach Appraisal forms assist the BOD with new and returning Coach Selection, determining SRN Program direction & goals.
These forms are designed to collect all feedback. Did you have a good Coach? We need to hear from you! How did he accomplish his goals? This is helpful for other Coaches. This is great way to thank your Coach! Good news for our program!

Medical Release Form: This form is intended for Team Managers to hand out to team parents. Parents who may have misplaced their copy. Parent note: please return to your Team Manager ASAP.

Team Manager is responsible for the record; all forms must be shredded last game of the season. All information on this form is kept confidential, for SRN League use, player participation.
Little League Volunteer Forms: A condition of SRNLL Charter with Little League International All forms are kept confidential and shredded end of each year September prior to the elections.

Notes about the Volunteer Form:
Per Little League Guidelines all SRNLL Volunteers must complete this form, submit directly to mail to SRN League Secretary by US mail. This includes intermittent adult volunteers, on or off SRN property, at team practices, during all games, SRN events, Scorekeepers, Assistant Coaches, Coach Helpers Team Parents, umpires etc.
Board of Director and Coach Candidates, are elected and not considered legitimate candidates until the League Secretary has completed a background check.
Verbally telling someone about your intentions to volunteer or giving your volunteer application to anyone other than the League Secretary or Executive of SRNLL Board does not make the volunteer form complete.
